Medical documents play a critical role in ensuring individuals receive the care and legal protection they need. Whether it’s an Advanced Healthcare Directive, Medical Power of Attorney, or a Living Will, proper notarization is required to make these documents legally enforceable.

Are you or a loved one in a Hospital, Nursing Home, Assisted Living or Hospice Care?
Most health care facilities have visiting hours where Notaries can come. Please check with the nurses station or your assigned Social Worker on what the visiting hours for that facility.
Hospital Mobile Notaries are often needed by patients or employees of healthcare facilities, including hospitals, rehab centers and nursing homes. Common documents seen are the Medical or Financial power of attorney, or last will and living will.
We understand that patients who need to sign important documents are at their most vulnerable, could be very ill or medicated, and that these circumstances can be very emotional for the signer and their family.
In this setting you need a notary who is capable of identifying the patient while being sensitive and respectful to their unique situation.
Please remember that we are not attorneys. Notaries can not provide or prepare legal documents, can not offer legal advice, or provide any type of legal opinions. We strongly suggest that the patient’s family reach out to a lawyer for their required documents.
We will print general documents if needed- but we can not recommend, provide, or fill-in legal documents or templates.
Please consider the following requirements prior to schedule a mobile notary:
Documents must be filled in completely for notarization to occur.
The patient may be heavily medicated or have issues with alertness. Try to schedule the hospital notary at a time that the patient is feeling their best, and is awake and cognizant of their surroundings.
Signers must have identification, be willing to sign their documents and have knowledge of what they are signing. If the signer is a senior with expired identification, plan to provide two non-partial witnesses to vouch for the signer’s identity.




Common Concerns
We understand that these are difficult circumstances, and signer may be emotional. An important aspect of working with a signer in these conditions is to determine their exact wishes related to the documents. We must positively identify the signer and be certain that the documents are indeed something that the signer is willing to sign, understands and knows their content, and is not acting under duress.
YES. We can work with patients who do not have total mobility of their hands and who may be challenged with signing their name.
YES. We can work with patients who have suffered a stroke or who may not have their total speech function
NO. We cannot work with signers who are comatose.
NO. We cannot work with signers who are dementia.
The signer may be medicated or resting, but will need to be awake, alert and willing to sign the documents in the notary’s presence.
If unable to speak, determine if the signer can communicate with our notary using eye or hand signals, or by uttering sounds in response to questions.
In cases where the signer no longer has a valid driver’s license, (perhaps for an elderly signer who has not driven in years), Texas law allows using a Credible Witness who can vouch for the identity of the signer. Discuss this with the notary when setting the appointment.
Full payment is required up front before any transaction takes place.
If the notarization is unsuccessful for some unforeseen reason, then the signature fees will be refunded only, not the traveling fee to hospital, nursing home or rehab.
We will also need a current, unexpired government issued picture ID (driver license, state ID, passport) for the signer.

Yes, it is a common practice in the mobile notary industry. If a notary travels to a location and the signer cancels, the notary may charge 100% of the travel and printing fees. These fees are typically a fixed cost, and the notary will have incurred expenses (travel time, gas) even if the notarization doesn't happen.
